About Us

PACE® provides comprehensive healthcare and social services to older adults (participants) still living independently in the community. Most of the participants who are in PACE® are dually eligible for both Medicare and Medicaid.

PACE® participants receive comprehensive, coordinated, customized care across various settings, including their homes and the Immanuel Pathways Center. PACE® is both a healthcare provider and a health plan.

Immanuel Pathways, our PACE® program, provides the entire continuum of medical care and long-term services and support required by older adults. These include primary and specialty medical care, such as audiology, dentistry, optometry, podiatry; homecare; prescriptions; specialty care; respite care; transportation; adult day services, including meals, nutritional counseling, social work, personal care, and physical, occupational and recreational therapies; as well as hospital and nursing home care, when necessary.

PACE® covers all Medicare Parts A, B, and D benefits, all Medicaid-covered benefits, and any other services or support that is medically necessary to maintain or improve the health and well-being of PACE® participants.

Medicaid-eligible individuals pay no premium for Immanuel Pathways services.
